目的:了解2020年赤峰市管理的艾滋病患者CD4+T淋巴细胞(简称CD4细胞)检测情况并探讨其相关影响因素,为制订针对性防控措施提供依据。方法:以赤峰市管理的766例HIV感染者为研究对象,采集抗凝血样本,用流式细胞仪进行CD4细胞检测,于中国疾病预防控制信息系统艾滋病防治基本信息系统收集HIV感染者基本资料,应用二元Logistic回归进行CD4细胞检测情况及影响因素分析。结果:2020年赤峰市CD4细胞检测率为90.73 %(695/766);CD4细胞计数为(686.31±352.7)个/μL;Logistic多因素回归分析发现,职业为家政及待业、商业服务、农民、学生、干部,已婚,诊断时间为2011年以后,样本来源为艾滋病自愿检测咨询(voluntary counselling and testing,VCT)、其他就诊者、术前检测的患者更容易进行CD4细胞检测(P＜0.05),病程尚处于HIV感染阶段是降低CD4细胞检测比例的影响因素(P＜0.05)。结论:病程尚处于HIV感染阶段患者CD4细胞检测率较低,在今后的检测工作中,要针对该人群的特点采取针对性措施,加强检测依从性教育,从而控制人类免疫缺陷病毒的传播扩散。
Objective: To study the detection of CD4+T lymphocytes (CD4 cells) in AIDS patients of Chifeng City in 2020 and explore the related influencing factors, with the purpose of providing references for making prevention and control measures. Methods: A total of 766 HIV/AIDS patients in Chifeng City were selected as research objects,anticoagulant blood samples were collected and CD4 cells were detected using flow cytometry. The basic data of HIV/AIDS patients were collected from the Basic Information System for AIDS Prevention and Control in Disease Prevention and Control Information System of China, and the influence factors in CD4 cell detection were analyzed using binary logistic regression. Results: The results showed that the detection rate of CD4 cells was 90.73 % (695/766) in AIDS patients in Chifeng City in 2020. Logistic multivariate regression analysis results suggested that the population susceptible to CD4 cell testing were unemployed workers, commercial service workers, farmers, students and cadres, and married patients, patients who diagnosed after 2011, samples came from HIV voluntary counselling and testing (VCT), patients who underwent preoperative testing and other medical visits. The main influencing factor of decreasing the proportion of CD4 cells testing were HIV infection stage. Conclusion: The results showed that CD4 cell detection rate was lower in HIV patients under infection stage. The detection work in the future should be targeted on HIV patients under infection stage, to control the HIV spreading by strengthening the detection compliance education on these patients.
