Objective: This study aimed to investigate the clinical significance of neutrophil percentage-to-albumin ratio (NPAR) in evaluating degree of coronary artery lesion and predicting prognosis of patients with coronary heart disease (CHD). Methods: A case-control study was conducted, enrolling 112 CHD patients admitted to Zhengzhou Second People's Hospital (September 2020-October 2023) and 112 age-/gender-matched healthy controls. Participants were stratified into high (n=70) and low (n=42) severity subgroups based on Gensini scores. Clinical parameters were systematically collected, including demographic characteristics, laboratory biomarkers, and imaging data. Six-month readmission outcomes were tracked to establish prognostic subgroups. Spearman's correlation analysis assessed NPAR-Gensini score associations, while multivariate logistic regression models identified independent predictors of severe coronary lesions. Results: CHD patients exhibited significantly elevated NPAR levels compared to controls (P<0.05). Stratified analysis revealed higher NPAR values in the high Gensini score subgroup versus the low-score subgroup (P<0.05), and in readmission cases versus non-readmission cases (P<0.05). NPAR demonstrated a strong positive correlation with Gensini scores (r=0.611, (P<0.001). Multivariate analysis identified NPAR as an independent risk predictor for severe coronary lesions (OR=6.037, 95% CI: 3.527-9.724). Conclusion: Elevated NPAR levels are closely associated with advanced coronary atherosclerosis and adverse clinical outcomes in CHD patients. This biomarker shows promise as a cost-effective prognostic tool for risk stratification and readmission prediction in CHD management.
