Objective: To investigate the correlation between lipoprotein (a)[Lp(a)] and clinical outcome in patients with acute ischemic stroke with large artery occlusion in anterior circulation undergoing endovascular treatment. Methods: Patients diagnosed with acute ischemic stroke (AIS) with anterior circulation large artery occlusion within 24 hours of onset and treated with endovascular treatment (EVT) in the Department of Neurology, Baotou Central Hospital were enrolled. The baseline data, blood test indexes, operation related information, and prognosis of the patients were recorded in detail. The outcome was the proportion of patients with a good outcome at 3 months after EVT, defined as a modified Rankin scale (mRs) score of 0-2. SPSS 25.0 statistical software was used to analyze all collected data to further evaluate the correlation between Lp (a) and clinical outcomes of EVT patients. Results: A total of 265 patients were enrolled, including 141 patients (53.2%) in the good prognosis group and 124 patients (46.8%) in the poor prognosis group. The Lp(a) level in the good prognosis group was lower than that in the poor prognosis group, and the difference was statistically significant. Multifactor logistic regression analysis showed Lp(a) OR=1.032; 95%CI=1.017-1.047. ROC analysis of Lp (a) for poor outcomes in EVT patients showed that the AUC was 0.655 (95%CI=0.590-0.721, P<0.001), the predictive cut-off value was 20.95 mg/dL, the sensitivity was 77.4%, and the specificity was 51.1%. Conclusion: Lp (a) level is related to the clinical outcome at 3 months after EVT in patients with acute ischemic stroke with large artery occlusion in the anterior circulation, and has certain predictive value for its clinical outcome.
