探讨CA125、PA、CHE和RDW联合检测评价肝硬化患者肝功能的价值

摘要

目的: 探讨糖类抗原125(CA125)、前白蛋白(PA)、胆碱酯酶(CHE)和红细胞分布宽度(RDW)评价肝硬化患者肝功能的价值。方法: 随机选取2023年6月至2024年10月于包头市中心医院消化科初诊初治肝硬化患者80例,根据肝硬化分期将研究组分为代偿期组(25例),失代偿期组(55例),对照组选取体检中心体检者的血清标本100例,比较不同分组之间的CA125、PA、CHE和RDW,采用ROC曲线下面积(AUC)评估CA125、PA、CHE和RDW单项或联合检测对肝硬化患者肝功能的评估价值;采用Spearman相关性分析法分析CA125、PA、CHE以及RDW水平与疾病严重程度相关性。结果: 研究组血清CA125、RDW水平高于对照组,而PA、CHE水平较对照组下降,差异具有统计学意义(P<0.05);ROC曲线结果显示,血清CA125、PA、CHE以及RDW联合检测的AUC值为0.96,高于其他单项检测的AUC;相关性分析显示,联合检测与疾病严重程度的相关性更强。结论: 不同时期肝硬化患者的CA125、PA、CHE和RDW水平存在明显差异,联合检测对评估肝硬化患者肝功能具有更高价值。

Abstract

Objective: To investigate the value of CA125, PA, CHE and RDW in evaluating liver function in patients with liver cirrhosis. Methods: A total of 80 patients with liver cirrhosis who were first diagnosed and treated in the Department of Gastroenterology of Baotou Central Hospital from June 2023 to October 2024 were randomly selected. According to the period of liver cirrhosis, they were divided into compensatory group (25 patients) and decompensated group (55 patients). The serum samples of 100 patients with normal results were selected in the control group. The CA125, prealbumin (PA), cholinesterase (CHE) and red blood cell distribution width (RDW) were compared between the study group and the control group. The area under the ROC curve (AUC) was used to evaluate the value of single or combined detection of CA125, PA, CHE and RDW in evaluating liver function in patients with cirrhosis. Spearman correlation analysis was used to analyze the correlation between CA125, PA, CHE and RDW levels and disease severity. Results: The levels of serum CA125 and RDW in the study group were higher than those in the control group, while the levels of PA and CHE were lower than those in the control group, and the differences were statistically significant (P<0.05). ROC curve results showed that the AUC value of combined detection of serum CA125, PA, CHE and RDW was 0.96, which was higher than that of other single detections. Correlation analysis showed that the correlation between combined detection and disease severity was stronger (r=0.87) (P<0.05). Conclusion: There are significant differences in CA125, PA, CHE and RDW levels in patients with cirrhosis at different stages. Combined detection has a higher value in evaluating liver function in patients with cirrhosis.
